All clients/ youth participate in an individual psychosocial assessment processed by a counselor or counselor intern within 3 service days of admission. Current policy indicates that the assessment is completed to obtain enough information for the admission to justify the admission; cases of assessment being incomplete at the time of the admission are the exception. Duration and frequency of treatment is based upon the need of the client.
A counselor or counselor intern must conduct and document a comprehensive psychosocial assessment utilizing the SASS13 (a bried and easy to use screening measure that helps to identiy individual who have a high probablity of having a substance use disorder). The assessment must document as assess enough information about the client’s past and present status to provide a thorough understanding of the following areas.
- Alcohol and Drug Use
- Psychiatric and Chemical Dependency
- Co-occurring Conditions
- Medical History
- Relationships with Family
- Environment and Living Situation
- Reports of elder or child neglect or abuse
- Social and leisure activities culture
- Education and Vocational Training
- Religious and Spiritual Orientation
- Childhood History, Sexual History, Assault, Rape, and Domestic Violence
